This 3.5 hour virtual classroom workshop is designed to provide an overview of Personalised Care and Support Planning (PCSP) for people living with cancer including elements formerly known as the Recovery Package. There will be the opportunity to explore how PCSP can be implemented into your workplace. The session will provide a forum for discussion and debate, with opportunities to learn, share and network with other professionals.
During this session we aim to help you:
• Understand strategic drivers regarding personalised care and support for people living with cancer.
• Understand the key components of personalised care including holistic needs assessment, cancer care reviews, treatment summaries and health & wellbeing information and support.
• Explore a range of approaches and strategies to promote effective implementation of personalised care and support planning/Recovery Package.
• Demonstrate use of a quality improvement tool as a framework for implementing personalised care and support planning.
Who is it for> This session is at an introductory level and is for everyone involved with implementing personalised care and support planning, including holistic needs assessment, cancer care reviews, treatment summaries and health & wellbeing information and support at any level. You may be in a registered or un-registered clinical post, information and support role or a managerial or project post. It is particularly aimed at those who are new to planning, managing or implementing personalised care and support planning elements for people living with cancer.
