What is Personalised Care and Support Planning? How does the Recovery Package support it?

Personalised care means people have choice and control over the way their care is planned and delivered.

It is based on what matters to them and their individual strengths and needs. It is fundamental to delivering better quality integrated care, that can be meaningful to the individual, facilitating improvements in health and wellbeing.

The Recovery Package is a series of key interventions which, when delivered together, can greatly improve outcomes for people living with and beyond cancer.

The Recovery Package is made up of 4 direct components:

  • Holistic Needs Assessment, Care and Support Planning
  • Treatment Summary
  • Health and Wellbeing
  • Cancer Care Review
