This category focuses on high-performing teams who have developed their roles, relationships and processes to create high-quality and efficient service delivery.

We're pleased to introduce one of the winning teams from last year, The Devon Macmillan GP Facilitator Team at North East and West Devon CCG and Torbay and South Devon CCG, through creating influential partnerships across health and social care they've developed and established tools to aid the delivery of the best end of life care to patients in their chosen care setting.
